What We Do


CIS currently serves students at four Nassau County schools.
  • Callahan Middle School
  • Fernandina Beach Middle School
  • Hilliard Middle-Senior High School
  • Yulee Middle School

As a partner, CIS works inside the schools at two levels of intensity, using an evidence-based model to match the degree of intervention to the scale of risk factors.



School-wide CIS Services are short-term preventative measures provided and sourced by CIS in alignment with the school improvement plan. Students do not need to be enrolled in a specific CIS initiative to benefit from these services, but simply need to be enrolled at the school. Examples: clothing, food, eyeglasses, school supplies, short-term homework help, walk-in career coaching, guest speakers, or part-time job assistance.

Focused Case Management Services are directed to students with specific needs. They are designed to achieve measurable outcomes, including: improved grades, FSA scores, attendance and/or behavior and are measured pre and post service delivery.

Case Management Process:
  1. Student is referred for CIS services by school or parent/guardian
  2. School-based CIS coordinator meets with student and parent/guardian to discuss challenges and additional needs
  3. CIS coordinator works individually with the student to set realistic goals and action plans for improvement
  4. Prescriptive services provided directly and/or coordinated at the school and/or sourced in the community
  5. Site coordinator monitors student performance and meets individually to coach and reinforce student progress
